Threshold’s Floating Support service, funded by Supporting People (NIHE), provides tailored support for individuals with mental health challenges. Support is available Monday to Friday, 9-5. The service can last up to two years, depending on the individual’s needs. Each person is assigned a keyworker who helps design a personalised support plan. This may involve weekly home visits for practical support or meetings in a coffee shop to encourage social engagement. Regular reviews are held at 3, 12, and 21 months to assess progress and adapt the support to meet changing needs.

Eligibility
You are eligible for our service at Callaghan Court if you meet all of the following conditions:
- Your primary diagnosis is mental health
- You are over 18 years’ old
- You have a mental health team
- You have been self-medicating for 6 months
- You have had a reduction in self-harm within the past 6 months
- You think that you need support to stay out of hospital and are working towards living independently in the wider community
- Your Social Worker, Community Nurse or Doctor agrees with your choice to access this service
- You agree to work within the licence agreement agreed between Choice and Threshold
