Each person is assigned a keyworker who helps create a personalised support plan. Support may include weekly home visits for practical assistance or meetings in community spaces, such as coffee shops, to encourage social engagement. Regular reviews are undertaken every 3 months to ensure the service adapts to the person’s changing needs and to discuss the progress of their support.

Who can apply

  • Over 18 years old
  • Have a Physical disability, Sensory Impairment and/or Acquired Brian Injury.
  • Home owner or tenant of private or social housing
  • Have support needs that can be met by the service
  • Live within the Southern Trust catchment area. 

Referrals

Referrals can be made either by the individual (self-referral) or by a healthcare professional such as a Social Worker, GP, or other relevant practitioner. A referral form must be completed in full and can be requested or submitted via email to lurganfloatingsupport@threshold-services.co.uk.
